Dear Parents

The Haldimand-Norfolk Health Unit in collaboration with several agencies in Haldimand and Norfolk want to determine the needs of parents with children 0-18 years of age. We want to find out about parenting services and programs that are important to you.

In order to do this, we are asking parents of children 0-18 in Haldimand and Norfolk to complete this short survey. This will take approximately 10 minutes.

This information will help to determine programs and services needed by parents in our community. Your opinions will help us improve our programs to better meet your parenting needs.
• Participation is completely voluntary and your individual information will remain confidential and anonymous.
• You can refuse to answer any questions.
• Please do not put your name on the survey.
